    • Syllabus Materials Upload Form

      Please provide a copy of your lecture slides for the conference syllabus - even if you plan to update or amend them prior to your presentation. The syllabus will be provided to participants on a CD; therefore, these materials MUST be submitted electronically. We will accept PowerPoint slides, in PPT format (no PDF's) or documents in Microsoft Word format. Submit your slides by the upload form or email to Joye Stewart at joye@societyhq.com.

    • Needs Assessment

      In order to provide Category 1-A CME credit to conference participants, the American Osteopathic Association requires needs assessment documentation for each lecture. Please refer to your speaker letter for more details on this required information.

    • Audio Visual Information

      The Academy will provide a lavalier microphone, laser pointer, screen, LCD projector and computer with Microsoft capability. The conference room will utilize the in-house sound system. If you use a MAC, please be sure to bring your computer with you including the appropriate connector for the LCD projector. Mac users, if you decide to bring your presentation on a thumb drive, please realize that certain elements of your slides may not convert when viewed on the computer provided. If you have additional audio visual needs, please contact Joye Stewart at joye@societyhq.com with your request.

    • Registration and CME (Deadline Friday, September 24, 2010)

      If you wish to receive CME credit (DO speakers only) for the course in addition to your presentation, please submit your registration form and payment to the AOA. Registration is complimentary for our allopathic guest speakers and for those who do not wish to receive CME credit. This course has not been approved to provide CME credit for allopathic physicians. If you will not receive CME credit and would like complimentary registration, please note this on your Speaker Credentials Form. The printable registration form. Please be sure to check the "Orthopedics" box when completing your form.

    • Hotel Reservations (Deadline Friday, September 24, 2010)

      Speakers are responsible for making their own hotel reservations. The AOAO's headquarters hotel is the Marriott San Francisco Marquis. Reservations may be made through the AOA housing bureau.
